Kinds of autism, and how it shows up differently
Since 2013 there is officially one diagnosis — autism spectrum disorder — rather than the separate labels that came before. But families still meet the old terms, and real differences between autistic people matter enormously in practice.
The older labels you'll still hear

Profiles you'll recognise in real children
None of these are diagnoses. They're patterns that help parents make sense of what they're seeing.
The child who speaks a lot but struggles socially
Fluent, articulate, sometimes strikingly knowledgeable — and still lost in the social rules of a playground. Because speech is intact, support is often denied on the grounds that they're "doing fine academically." Anxiety, exhaustion after school, and meltdowns that only happen at home are the usual signs that they are not.
The non-speaking or minimally speaking child
Roughly a quarter to a third of autistic people use little or no speech. Non-speaking is not the same as having nothing to say and is not a measure of intelligence. Augmentative and alternative communication — picture systems, sign, speech-generating devices, typing — should be introduced early. Research is consistent that AAC does not stop speech from developing; if anything it supports it.
Autistic girls and the missed-diagnosis problem
Girls are diagnosed later and less often, partly because diagnostic criteria were built on studies of boys. Interests may look socially typical on the surface — animals, books, a band — but are pursued with autistic intensity. Social difficulty may be hidden by copying a friend closely. Many are diagnosed with anxiety or an eating disorder first, and identified as autistic only in adolescence or adulthood.
The child with high support needs
Requires substantial daily help with communication, safety and self-care, often alongside intellectual disability, epilepsy or self-injury. These families need practical things — respite, waiver funding, safety equipment, skilled behavioural and medical support — and often feel talked over in public conversations about autism. Their needs are not in competition with anyone else's.
The late-identified parent
It is very common for a parent to recognise themselves partway through their child's evaluation. Autism is substantially heritable. Adult diagnosis is expensive and hard to access, and a lot of people stop at self-identification, which the autistic community broadly accepts.
Regression
Some children develop words and social skills and then lose them, usually between 15 and 30 months. This is a real and documented pattern, and it is frightening to live through. It is not caused by anything you did, and the timing coincidentally overlaps with the routine vaccination schedule — which is a large part of why a false link took hold.

Perfiles que reconocerá en niños reales
Ninguno de estos es un diagnóstico. Son patrones que ayudan a los padres a entender lo que están viendo.
El niño que habla mucho pero le cuesta socialmente
Fluido, elocuente, a veces con conocimientos sorprendentes, y aun así perdido ante las reglas sociales del patio. Como el habla está intacta, a menudo se le niegan apoyos con el argumento de que «va bien académicamente». La ansiedad, el agotamiento al salir de clase y las crisis que solo ocurren en casa suelen ser la señal de que no va bien.
El niño no hablante o con habla mínima
Entre una cuarta parte y un tercio de las personas autistas usan poco o ningún habla. No hablar no es lo mismo que no tener nada que decir, y no mide la inteligencia. La comunicación aumentativa y alternativa —pictogramas, signos, dispositivos que generan voz, escritura— debe introducirse pronto. La investigación es consistente: la CAA no impide que aparezca el habla; más bien la favorece.
Las niñas autistas y el problema del diagnóstico perdido
Las niñas se diagnostican más tarde y con menos frecuencia, en parte porque los criterios se construyeron sobre estudios hechos con niños. Sus intereses pueden parecer socialmente típicos por fuera —animales, libros, un grupo de música— pero se viven con intensidad autista. La dificultad social puede quedar oculta copiando de cerca a una amiga. A muchas se las diagnostica antes de ansiedad o de un trastorno alimentario, y se las identifica como autistas solo en la adolescencia o en la edad adulta.
El niño con altas necesidades de apoyo
Necesita ayuda sustancial cada día con la comunicación, la seguridad y el autocuidado, a menudo junto con discapacidad intelectual, epilepsia o autolesiones. Estas familias necesitan cosas concretas —relevo, financiación por waiver, equipamiento de seguridad, apoyo conductual y médico competente— y con frecuencia sienten que se habla por encima de ellas en el debate público sobre el autismo. Sus necesidades no compiten con las de nadie.
El padre o la madre identificado tarde
Es muy común que un progenitor se reconozca a sí mismo a mitad de la evaluación de su hijo. El autismo es en gran medida hereditario. El diagnóstico adulto es caro y difícil de conseguir, y mucha gente se queda en la autoidentificación, algo que la comunidad autista acepta de forma generalizada.
La regresión
Algunos niños desarrollan palabras y habilidades sociales y después las pierden, normalmente entre los 15 y los 30 meses. Es un patrón real y documentado, y da mucho miedo vivirlo. No lo causó nada que usted hiciera, y coincide por casualidad con el calendario habitual de vacunación, lo cual explica en buena parte por qué arraigó un vínculo falso.
